About Us

Advance Care Management was founded in 2018, in Lubbock, Texas, as a solution to healthcare providers who wanted to provide Chronic Care Management services to their Medicare populations but did not have the time or resources to implement those services in-house.

Over the past 3 years, Advance Care Management has assisted clients in West Texas and the Texas Panhandle better manage their patients with chronic illnesses while also assisting communication between patient and physician.

 

As a market leader in CCM and RPM in the Texas Panhandle, we have the experience and a level of customer service that no other CCM/RPM provider in our region can match.
