Lubbock's fastest growing and top chronic care management company currently has an opening in our Lubbock office for a LVN Care Manager. This is a new position to lead a team of Medical Assistants.

This is a full-time position and responsibilities include, but are not limited to:

• Contacting patients by phone to discuss their current medical condition(s) and needs

• Creating care plans

• Working directly with physician's offices

• Overseeing a team of Medical Assistants
